Building state-wide capacity for integrated, front-line responses to people with multiple needs who are, or at risk of homelessness
The Queensland Housing Strategy 2017-27 is driving key reforms and targeted investment to create better housing pathways, housing growth and improved service delivery. Through Partnering for Impact and Partnering for Growth, the Department of Housing and Public Works (the department) is partnering with the homelessness and broader service sector as well as other government agencies to create better outcomes for people who are homeless or at the risk of homelessness by enhancing current service delivery offerings and encouraging innovation in transforming the housing and homelessness sector.
The department has contracted Q Shelter to lead the 12 month Queensland Service Integration initiative in providing a state-wide backbone support role to nine priority locations in strengthening existing networking and care coordination structures. The backbone support role provides leadership, facilitation and support to each location’s care coordination facilitator in reviewing, enhancing or developing robust practice frameworks, tools and resources. The backbone support role will lead a capability building program to support the continued development of the sector’s workforce, as well as design and undertake a robust mixed methods evaluation by utilising data captured from each priority location.
The care coordination facilitator role is responsible for providing place-based support to the existing care coordination and networking groups across each location, with a specific focus on enhancing person-centred care coordination for people with multiple needs that are, or at risk of homelessness. The position will seek to enhance engagement, collaboration and capability building opportunities in creating a more integrated and coordinated service response mechanism. Aligned with the evaluation process, the role will ensure that high quality qualitative and quantitative data is captured.
